Provider Enrollment Application Fee
42 CFR 424.514 requires that certain providers/suppliers pay the Provider Enrollment Application Fee. Providers/Suppliers whose enrollment applications meet the criteria for a fee will be prompted to submit payment during the submission process of their Internet application. Alternately, providers/suppliers may choose to submit payment outside of their Internet application by directly accessing the following link:
